To find a location, use the navigation tool on the right.  Simply select the type of care needed for you or your loved one from the “Find care near you:” drop down box. Then enter the area in which you are looking for care.  Either enter the city and state or the zip code and click “GO.” This will redirect you to a page containing a list of the closest facilities within the specified area.  If the search draws no results, that means there are no facilities within the given area.

If you are not sure which type of care is needed, use the two drop down boxes in the bottom section titled, “Help determine your care needs:”  Once you select your choices from both boxes, click “GO” and you will be redirected to a page describing the type of care needed.
