Home Care

Heartland Home Health Care provides advanced medical care combined with a compassionate touch, and allows patients to remain in the environment where they are most comfortable – their own home.

If so, Heartland has answers. Heartland’s care and services are coordinated and supervised by a registered nurse or therapist, and provided in the comfort of our patients’ homes. Our mission is called enriching life, and the Heartland team is focused on enriching our patients’ lives by helping them restore their independence and once again feel as if they are in control of their lives.

Our care team:
•   Provides comprehensive skilled nursing care, therapy services and I.V. therapy.
•   Educates patients, families and caregivers on disease self-management and newly diagnosed diseases such as diabetes, Alzheimer’s disease, heart disease and chronic obstructive pulmonary disease.
•   Assists in medication management including assessing new medication, changes in frequency, dosage and interactions with other medications.
•   Performs therapy service to help patients recovering after surgery, illness or injury such as joint replacement or stroke.

If a patient is experiencing an illness, hospitalization or surgery and requires recovery and healing, or if he or she is facing a new health care diagnosis requiring a change in daily lifestyle, home health care might be an appropriate solution. Heartland provides support to patients and family members by identifying goals and potential obstacles, and helping the patient recover, understand and identify symptoms, manage medication, and learn to deal with anxiety, fear or other feelings that come with illness.

Receiving home health care

We offer help and support to patients and families 24 hours day, seven days a week. Those eligible under Medicare’s home health care guidelines include:

•   Individuals who have difficulty leaving the home.
•   Individuals who require skilled nursing or skilled therapy services on a part-time basis.
•   Individuals under a doctor’s care plan.

Heartland will contact your physician to ensure that you are eligible for the home health care benefit.

Paying for home health care

Home health care is usually covered under Medicare, Medicaid and many health care plans. Our administrative team is available to verify your benefits, coordinate coverage and complete the billing process.
