Hospice isn’t a place.  It’s a philosophy of care.  Our mission is called enriching life.

Heartland Hospice is dedicated to providing patients with care wherever they consider home. Whether care is desired in the home a patient has lived in for years, a skilled nursing center or an assisted living facility, we tailor our caregiving to fit the patient’s and family’s unique needs.  

Heartland’s caregivers enrich our patients’ lives and provide support by developing individualized care plans, setting goals, encouraging personal choices, working together in caregiving decisions and supporting meaningful experiences.  Our caregivers help manage pain and symptoms, enabling patients to live life as fully and comfortably as possible.  Social workers and spiritual counselors create a comforting environment in which patients and families are able to share their thoughts, hopes and concerns.  And special bereavement programs provide emotional support to patients and families who need help coping.

Heartland Hospice supports the patient and family wherever they are in their health care journey, and upholds the patient’s and family’s wishes, goals and values.  At Heartland, we help preserve our patients’ dignity, show them the respect that they richly deserve and provide compassionate care through our Caring Promises.

•   Patient’s physician and hospice physician
•   Registered nurses
•   Social workers
•   Spiritual care coordinators
•   Bereavement counselors
•   Dietitians
•   Pharmacists
•   Physical, occupational and speech therapists
•   Hospice aides
•   Volunteers



Admission criteria

Hospice care is available to patients with an advanced illness.  Medicare’s guidelines include both of the following:

•   Documented life-limiting prognosis by a certified physician(s)
•   The patient elects to receive comfort (palliative) care  

Heartland will contact your physician to ensure that you are eligible for the hospice benefit.  We offer support to patients and families 24 hours a day, seven days a week.


Paying for hospice care

Hospice is a 100 percent-covered benefit under Medicare and covers the interdisciplinary team’s services, medications, equipment and supplies related to the patient’s life-limiting illness. Hospice care is also covered under Medicaid and many insurance plans.

Heartland Hospice provides care to those who qualify for hospice services regardless of the ability to pay or their insurance coverage.
