Heartland Health Care Center - Paxton is a proven leader in skilled nursing and rehabilitation, located on Illinois State Route 9. We are just 3 miles from the State Route 9 interjection with Illinois Interstate 57 and 1 mile east of the Illinois Route 9 interjection with Illinois Route 45.

Heartland provides a wide range of comprehensive medical services including 24-hour nursing care and physical, occupational and speech therapy to a broad spectrum of patients. In addition to our skilled nursing and rehabilitation services, we offer therapeutic recreation and social activities.

The majority of our rehabilitation patients return home to lead productive lives, however for patients looking for long-term care, our professional staff provides comprehensive health care around-the-clock in a safe and comfortable environment.
