Welcome to Heartland of Mentor, located on a beautiful wooded setting off of Route 615 just south of Route 90. This state-of-the-art center specializes in short-term post-acute care. Our highly trained physical, occupational and speech therapists provide individualized care plans to meet the needs of each patient. Heartland also provides respiratory therapy. Our experienced nursing team provides care for clinically complex patients in both private and semi-private rooms.

The social work team is active with patients to assist with discharge plans in order to help patients return to the most independent level of living. Heartland provides many home-like lounges for comfortable family and friend visitation, including WiFi service for patients during their stay at Heartland of Mentor. Dietitians also work in conjunction with our food service manager to provide wholesome, tasty meals in our on-site kitchen. Heartland of Mentor is Medicare certified and is also contracted with many private insurances.
