Patricia suffered a slight stroke and had fallen at home.
In order for Patricia to go home independently and down a few stairs to her apartment she knew she would need physical and occupational therapy.
At Heartland Health Care Center - Knollview, the interdisciplinary team created a care plan to help Patricia reach her goals. The therapy team worked with her on balance and stairs in order for Patricia to return to her apartment with steps safely. Also, the nursing team helped Patricia manage her pain while going through therapy.
